Occupational/Physical Therapy Scholarship is awarded to two students who are in financial need and have been accepted or are attending an accredited school of occupational therapy (including art, music, or physical therapy). A letter of acceptance into the program or the transcript noting the applicant is enrolled in the program must be included with the application. Applicants must submit a proof of United States citizenship. At least two, but not more than four recommendations and a one-page resume must also be submitted.
